The purpose of the ADHD assessment is to collect comprehensive information about your behavioral, cognitive, and emotional functioning. This helps identify patterns that may indicate Attention Deficit Hyperactivity Disorder (ADHD) or related conditions.
Please note: this assessment is not a formal diagnosis. It is a structured process designed to inform the clinical judgment of a licensed healthcare provider, such as a Nurse Practitioner within So You Need Therapy, who will make the final diagnostic decision based on a full evaluation.
To ensure a meaningful and accurate assessment, clients are expected to provide honest and complete information throughout the process. This includes all questionnaires, observer forms, and interview responses.
All personal information shared during the ADHD assessment process—including written and verbal content—will remain confidential, in accordance with professional standards and applicable privacy legislation. Exceptions to confidentiality may apply if there is a risk of harm to self or others, or when legally required to report.
In such situations, efforts will be made to inform you if a breach of confidentiality becomes necessary.
Your assessment results may be shared with a Nurse Practitioner or other healthcare professionals within So You Need Therapy, solely for the purpose of supporting diagnostic clarity and care planning. Information is shared securely and only with your consent.
The assessment may involve discussing sensitive personal experiences, which could feel uncomfortable or emotionally triggering for some individuals.

Clients must be physically located in Ontario, Canada at the time of the assessment. This is necessary to comply with the legal and regulatory standards under which our licensed healthcare providers operate.
If you are not located in Ontario during your session, it may be canceled or postponed without refund.
